Step-by-Step Process
Check Eligibility
On the PM-JAY 'Am I Eligible' portal or mParivar Vahini app, enter your mobile number, state, and ration card/name to check if your family is listed under the SECC 2011 Beneficiary Identification System (BIS).
Visit CSC or Empanelled Hospital
If eligible, visit a Common Service Centre (CSC), empanelled hospital's Ayushman Mitra desk, or nearby CSC/Ayushman kiosk to complete e-KYC using your Aadhaar and mobile OTP.
Biometric/OTP Verification
The operator verifies each family member's identity via fingerprint scan or Aadhaar OTP, and uploads a live photo for the card.
Card Approval and Printing
The application is sent for approval to the State Health Agency (SHA); once approved, the Ayushman card is generated instantly and can be printed or downloaded as PDF.
Use Card for Cashless Treatment
Present the Ayushman card and Aadhaar at any empanelled hospital's Ayushman Mitra desk for cashless hospitalisation covering pre-listed procedures up to Rs 5 lakh per family per year.

- Eligibility is based on SECC 2011 data and state-specific extensions - not everyone can enrol, so always check via the 'Am I Eligible' tool first.
- The Rs 5 lakh cover is per family per year and resets annually; it is on a floater basis, shared across all listed members.
