Step-by-Step Process
Register on Swavlamban Portal
Go to swavlambancard.gov.in, click 'Apply Online for UDID Card' and register with your name, mobile number, and state/district.
Fill Personal and Disability Details
Complete the online application form with personal details, disability type (as per the 21 categories under the RPwD Act), and upload a photo and identity proof.
Medical Board Assessment
Visit the assigned government hospital or district medical board on the appointment date for a physical examination to certify the type and percentage of disability.
Certificate Generation
Once the medical board uploads its assessment online, a disability certificate is auto-generated on the portal, viewable in your applicant login.
Download UDID Card
After certificate approval, download the UDID card (a plastic card is also posted by the state) which serves as proof of disability for all schemes and benefits nationwide.
